Hope City is thrilled to offer a Hope Tree opportunity for our church family to bless children in our community. We ask that if you have children that could be blessed by our Hope Tree, that you complete this form. You will complete a separate form for each child by clicking "Add another child registration" at the bottom of the form.

Please submit registrations by November 8th. Registrations received after November 8th will not be guaranteed to be included in the tree, depending on availability.
